ISSUANCE OF MARRIAGE LICENSE

ABOUT THE SERVICE:Before getting married each of the contracting parties must file separate sworn applications for Marriage License with the proper local civil registrar of the place where either or both of the contracting parties reside.

Marriage licenses are valid in any part of the Philippines for a period of 120 days from the date of issue.

They are deemed automatically cancelled if the contracting parties have not yet gotten married within this period.

REQUIREMENTS FOR APPLICATION OF MARRIAGE LICENSE

  1. Birth Certificate
  2. Parental Consent ( age 18 to 20 years old)
  3. Parental Advice ( age 21-24 years old)
  4. Certificate of Pre-marriage counseling
  5. Certificate of No Marriage ( CENOMAR)
  6. Certificate of Death ( if applicant is widow)
  7. Judicial decree of the absolute divorce or Judicial decree of annulment or declaration of nullity previous marriage ( if applicant had been  previously married)
  8. Affidavit of legal capacity to contract marriage to be issued by his diplomatic official. ( if applicant is a foreigner) or Certification that he has no embassy or consulate in the   Philippines to be obtained from Department of Foreign Affairs.  ( If the foreigner has no embassy or consulate in the Philippines.)

HOW TO AVAIL OF THE SERVICE:

1. Appear personally and accomplish the application for marriage license (Form No. 90, Form Review the accomplished application and enter the date when the application for marriage license was received 20 minutes Lolita T. De Vera

3. Go to the Treasurer’s Office and pay prescribed fee. Receive payment and issue Official Receipt. 10 minutes RCC II
3. Applicants sign their full name in the space provided for. The Civil Registrar administer the oath then sign over his printed name in the Form. 20 minutes Lolita T.De Vera
Post the notice of the application for marriage on a bulletin. 10 days MCR Staff

which is valid for 90 days

REGISTRATION OF MARRIAGE


ABOUT THE SERVICE:
For ordinary marriages, the time for submission of the Certificate of Marriage is 15 days following the solemnization of marriage. For marriages exempt from the license requirement, the prescribed period is 30 days.

REGISTRATION OF DEATH

ABOUT THE SERVICE: The death of a person must be registered within the 30 days reglamentary period at the office of the Municipal Civil Registrar.

REQUIREMENTS:

  • Certificate of Death (Form 103)

HOW TO AVAIL OF THE SERVICE:

1. Report the death of a person to the office of the Municipal Civil Registrar within 30 days from the time of death.  REGISTRATION OF BIRTH

 FEES         – P 50.00

 ABOUT THE SERVICE:The birth of a child, being a vital event for a person, must be registered within 30 days from the time of birth at the office of the Municipal Civil Registrar of the city/municipality where the birth occurred.The declaration of the physician , midwife or “hilot” in attendance at the birth or the declaration of either parent of the newborn child and the presentation of a marriage contract in cases of married parents shall be sufficient for the registration of a birth in the civil register.

REQUIREMENTS:

  • Certificate of Live Birth (COLB) Form 102
  • Birth Certificate of parents
  • Certificate of Marriage of Parents (if applicable)

HOW TO AVAIL OF THE SERVICE:

ABOUT THE SERVICE:Both parents must appear personally to the Office of the Municipal Civil Registrar and submit a copy of their  Marriage Certificate and the original copy of Live Birth of the Child.

REQUIREMENTS:

  • Certificate of Live Birth (COLB) – 3 photocopies
  • Marriage Contract – 3 photocopies
  • Community Tax Certificates of both parents

HOW TO AVAIL OF THE SERVICE:

ABOUT THE SERVICE : This service intends to enable the client to correct clerical error,  Change  the First Name  (RA9048)  and to correct the sex, and the date of birth of a child (RA10172). This service is performed without a judicial order.

WHO MAY AVAIL OF THE SERVICE:

  • Those civil events occurred in the municipality and registered in this office

NOTE: Processing for Affirmation of Petition at PSA is 3 months or more

  • Publication ( Published in newspaper of general circulation at least once a week for two consecutive weeks.) , copy of the newspaper clippings of the published petition and affidavit of publication from the publisher
  • Correction of Clerical Error (CCE)
  • Same requirements for the change of First name except Barangay, Police , NBI clearances and Publication
  • Correction of Sex and Date of Birth (RA10172)

  • Earliest school record. If never entered school, an affidavit attesting to the facts shall be submitted
