Issuance of Health, Medical Certificates and Other Related Permits

 ABOUT THE SERVICE: Issuance of health, medical certificates, sanitary permits, permit to open a tomb, transfers of cadaver.

REQUIREMENT/S:

  • For medical certificate:
  1. Complete medical history and physical examination by the examining physician
  2. Official receipt issued by the Office of the Municipal Treasurer
  • For health certificate (for employment)
  1. Laboratory test of the individual to be employed (urinalysis, sputum and stool)
  2. Specimens in a clean bottle and not in a cellophane or plastic bag
  3. ID picture (1 x 1 )
  4. Official receipt issued by the Office of the Municipal Treasurer
  • For sanitary permit/health certificates (for business)
  1. Laboratory test of the owner and his/her employees (urinalysis, sputum, and stool)
  2. Specimens in a clean bottle and not in a cellophane or plastic bag
  3. Official receipt issued by the Office of the Municipal Treasurer
  • For the transfer of cadaver/open a tomb
  1. Payment of fees and presentation of the official receipt issued by the municipal treasurers’ office and the death certificate issued by the municipal civil registry office.

Admission for Prenatal Examination

ABOUT THE SERVICE: All pregnant women are encourage to have their regular check-up to ensure the safety of the mother and the baby.

REQUIREMENT/S:

  • Mother’s home based chart

FEES: No fees required

HOW TO AVAIL OF THE SERVICE:

Maternal Delivery

ABOUT THE SERVICE: This service caters to all pregnant women experiencing labor pains and other symptoms related to maternal delivery.

REQUIREMENT/S:

  • Mother’s home based chart

FEES: No fees required

HOW TO AVAIL OF THE SERVICE:

1. Come to RHU with labor pains – Get the folder and review the previous visits

– Get the vital signs

– Perform the necessary procedures

– Observe the progress of labor (if complications arise conduct/assists patient to the hospital)

– Perform post natal procedure

Provision of immediate solutions to sanitation-related complaints

ABOUT THE SERVICE: The municipal government of Nueva Era through the municipal health officer receives and acts on complaints regarding sanitation problems and conducts immediate investigation.

REQUIREMENT/S:

  • Accomplished sanitary complaint form

HOW TO AVAIL OF THE SERVICE:

Provision of Dental Examination and Tooth Extraction Services

ABOUT THE SERVICE : Tooth extraction services are available every Monday and Wednesday only at the RHU Plus and Lying-in Clinic. This service is available to pre-school and school children, pregnant mothers and other adults to prevent and treat dental diseases.

REQUIREMENT/S:  None

HOW TO AVAIL OF THE SERVICE:

Provision of Anti-Rabies Vaccines

ABOUT THE SERVICE : The Municipal Health Office (MHO) provides anti-rabies vaccines for patients diagnosis of contracting rabies. The office provides vaccine for the first dose.

REQUIREMENT/S: None

FEES :

  • 1st dose free of charge
  • Succeeding doses to be shouldered by the patient

SERVICE SCHEDULES: (Personnel on duty on four shifts)

  • 24 hours on duly

HOW TO AVAIL OF THE SERVICE:

1. Approach the health personnel concerned and give information about the incident Interview patient 10 minutes Health Personnel on duty
2. Present self for anti-rabies injection Test the patient for possible allergic reaction 30 minutes Health Personnel on duly

Provision of Thermal Fogging

ABOUT THE SERVICE : One of the activities of the Municipal Government of Nueva Era in preventing the occurrence of dengue cases is the thermal fogging of public schools, day care centers, and other government buildings. Private schools, business establishments, households and the barangay may also avail of the free services but should provide the needed chemical and fuel.

REQUIREMENT/S:

  • Letter-request duly approved by the Municipal Mayor

HOW TO AVAIL OF THE SERVICE:

Provision of Immunization Services

ABOUT THE SERVICE : The Department of Health through the Municipal Health Office provides free immunization to children 0 to 11 months old from seven immunizable diseases.

The health personnel at the RHU and Barangay Health Station also minimize pregnant women to prevent the occurrence of tetanus neonatorum infants.

REQUIREMENT/S: None

HOW TO AVAIL OF THE SERVICE:

Provision of Anti-Tuberculosis Drugs

ABOUT THE SERVICE : The Department of Health through the Municipal Health Office (MHO) has an anti-tuberculosis program in the directly observed treatment short course chemotherapy (DOTS) Center at the RHU. The purpose is to identify and treat patients with tuberculosis (TB).

REQUIREMENT/S: None

FEES : Free of charge

HOW TO AVAIL OF THE SERVICE:

1. Ask for the requirements for the availment of free anti-TB drugs Inform the patient of the required laboratory examinations to be undertaken such as x-ray and sputum examinations 3 minutes Midwife on duty

Provision of Out-Patient Consultant at the RHU and BNSS

ABOUT THE SERVICE : The purpose of this service is to diagnose and treat illnesses and give appropriate medical services. The service is available at the municipal health office/RHUs/BNSs to any person/individual who needs medical assistance.

REQUIREMENT/S: None

FEES : Free of charge

HOW TO AVAIL OF THE SERVICE:
